My Participation as a speaker at NAHQ Next Conference 2017 – Session: 1004 HQ Talks. This session describes a process for organizing and analyzing incident report data. I described how I created a clear Excel dashboard that categorizes incidents based on their impact and ensures proper usage of all available data. I
analyzed incident reports at a national level as the main source for risk identification. Through dashboard tabs, we can determine the relation between:

• Impact and root causes
• Severity and root causes
• International patient safety goals and root causes
• Prioritization of decisions regarding severity, frequency, place, and impact
